An error occurred in the analyst at work article by Fonagy et al. (2004). Due to a technical problem involving the attachment containing Peter Fonagy's case material that was sent to Irwin Z. Hoffman for commentary, there was a discrepancy between the published version of Fonagy's case and the version that Hoffman received. This unfortunate error led Hoffman to include commentary that was based on some clinical material that did not appear in the published version.
