Divide su artículo en cuatro partes denominando a la primera “La Fantasía”. Allí describe una fantasía de una paciente de 31 años de edad, casada, estéril, frígida, y con dificultades conyugales. Básicamente, dicha fantasía concierne al origen de los sexos; para la paciente los niños nacían sin sexo determinado y era el padre, médico cirujano, quien durante el parto lo determinaba.
