Fenichel indicates the need for classification in psychosomatic conditions. Four classes of organ-neurotic symptoms are described: (1) affect equivalents; (2) results of disturbances in the chemistry of unsatisified persons; (3) physical results of unconscious attitudes; (4) combinations of the above. Most often these symptoms occur in combinations. The choice of organ depends on: (1) the nature of the instinctual demands that are warded off; (2) fixations due to the subject's earlier experiences; (3) the ability of the organs in question to express certain needs symbolically; (4) which organs had just been used or were specifically cathected at the moment when the decisive repression occurred; (5) the subject's previous physical history. A discussion of various syndromes is given which is helpful and clear. The author reveals his scepticism in regard to attempts to relate definite psychosomatic pictures to definite personality structures except in some few instances.
